Jenisha joined Triangle J Council of Governments in 2014 after completing an internship with the Triangle J Area Agency on Aging. Prior, Jenisha worked full-time as a physical therapist in the skilled nursing facilities throughout the Triangle region. Her professional experience is working with older adults and adults with disabilities.
- Provides person-centered counseling with interested residents in skilled nursing care to assist them in understanding existing home and community-based service options and to assist them in developing a plan for accessing needed services and supports and transitioning to community living.
- Provides education to long-term care residents, family members and key long-term care staff about home and community-based services and supports available to enable individuals' return to community living.
- Engages in outreach activities that inform the interested public about the available alternatives to institutional living.
- Identifies stakeholders necessary to build and maintain effective working relationships and foster community collaboration among identified partners, in order to facilitate successful transitions to community living for persons with disabilities.
